To Get Started, Follow These 3 Easy Steps:
Step 1: Fill Out the Online Registration and Consent Form
Visit our website and fill out the online registration form to officially sign up for our football program. Provide all necessary information accurately to ensure a smooth registration process.
Step 2: Make a Payment
After completing the online registration and consent form, proceed to make a secure online payment by clicking on the "Online Payment" button provided on our website. Follow the instructions to complete the payment process efficiently and securely.
Step 3: Upload Medical Clearance Form
Download and print the medical clearance form available on our website. Take the form to your physician for a medical evaluation and obtain clearance for participation in our football program. Once completed and signed by your physician, upload the form directly to our website using the designated upload portal.
